Use a status choice when a question is not applicable, unknown, or not assessed. A blank answer is never converted into a denial or normal finding.

Documentation principle: describe what the patient reported, what another historian reported, what EMS observed, what EMS assessed, what EMS did, and how the patient responded. Follow your agency’s approved abbreviations, local protocol, state requirements, medical direction, and ePCR rules.
